What to Expect as a New Patient
Scheduling & Referrals
To schedule your first appointment:
- Please arrange a referral from your primary care physician.
- Bring relevant medical records, including imaging (MRI, CT scans, etc.)—both the written reports and digital copies or films.
- Provide an up-to-date list of your current medications.
Insurance & Payment Options
Here at Lifespring Pain Management Center we:
- Insurance: We accept most major insurance plans. If you're unsure about coverage, please contact our office.
- Payment Methods: We accept cash and major credit cards for copays or self-pay patients.
If you experience an urgent issue related to a recent procedure:
- Contact our office during regular hours: 801-546-1300
- If you are unable to reach us, please call your primary care physician or visit the nearest emergency room.
Prescription Policy
Lifespring Pain Management Center follows strict guidelines for the safe and ethical use of prescription medications—especially controlled substances.
Important Prescription Guidelines:
- Controlled medications require a formal in-office visit and are typically prescribed one month at a time.
- Non-controlled medications may be eligible for electronic refills over the phone.
- Refills are not processed after hours or on weekends.
- Do not take more medication than prescribed.
- Lost or stolen prescriptions will not be replaced.
- All patients receiving controlled substances will be required to sign a medication agreement.
- Patients must comply with random urine drug screens and pill counts. Refusal or inconsistent results may lead to dismissal from our clinic.
These policies are in place to ensure your safety, comply with state and national regulations, and maintain the highest standard of care.
