What Is Lumbar Radiofrequency Neurotomy?

Also known as radiofrequency ablation (RFA), this procedure uses targeted heat energy to create a small lesion on the medial nerves—the nerves that carry pain signals from the facet joints of the lower spine. By disrupting these signals, your brain no longer registers the pain coming from the affected joints.

What to Expect During the Procedure

Preparation

You’ll lie face down on a treatment table. A sedative may be given to help you relax, and the injection area is sterilized and numbed with local anesthetic.


Needle & Electrode Placement

Using fluoroscopy (live X-ray), your physician carefully guides a thin cannula to the targeted medial nerve.


Testing the Nerve

A mild electrical current is delivered to confirm the correct nerve by briefly reproducing your pain or causing a muscle twitch.


Numbing & Heating

A numbing agent is applied to the nerve, then radiofrequency energy is used to heat the area and create a lesion that “turns off” the nerve’s pain signals.


Completion

The instruments are removed, and a bandage is applied. The entire procedure typically takes 30 to 60 minutes depending on the number of nerves treated.

After the Procedure
  • You’ll be monitored briefly before being discharged.
  • Have someone drive you home due to sedation.
  • Your back may feel sore or tender for several days.
  • Ice packs, rest, and light activity are recommended.
  • Pain relief may begin gradually, with full effects in 3–4 weeks as the nerves are fully disabled.

Your doctor may prescribe a short course of pain medication and provide aftercare instructions to ensure proper healing.

How Long Does Relief Last?
  • Most patients experience pain relief for 9 to 14 months, and some for up to 2 years
  • Treated nerves may regenerate over time—if pain returns, the procedure can be safely repeated
