What Is a Medial Branch Block?

A Medial Branch Block involves injecting a combination of local anesthetic and steroid medication near the medial branch nerves—tiny nerves that carry pain signals from the facet joints to the brain. If these joints become worn or damaged due to arthritis or injury, they can compress the nerves and cause persistent, localized pain.

If pain is relieved following the block, it strongly suggests that the facet joints are the source of the problem—making you a candidate for longer-lasting treatments like radiofrequency neurotomy.

What to Expect During the Procedure

Preparation

You’ll lie comfortably on an exam table. The treatment area will be cleaned and numbed with local anesthetic.


Imaging Guidance

Your physician will use ultrasound or fluoroscopy (real-time X-ray) to precisely locate the medial branch nerves.


Injection

A small needle is used to inject anesthetic and steroid medication into the targeted nerve area. You’ll be asked whether your pain has improved.


Evaluation

If pain decreases significantly during or after the procedure, it confirms that the facet joints are the source—helping guide further treatment.

The entire procedure usually takes 15–30 minutes and is performed on an outpatient basis.

After the Treatment
  • You may experience immediate relief due to the anesthetic, though this may wear off within a few hours.
  • The steroid medication may take a few days to begin working, at which point longer-term relief is expected.
  • You’ll be asked to rest and avoid strenuous activities for 24 hours after the procedure.
  • Soreness at the injection site is normal and usually resolves within a day or two.

Please have a family member or friend drive you home following the procedure due to the effects of sedation or numbness.
